What is a Physical Therapist?

Physical therapists are highly trained specialists who require a Masters and/or Doctorate Degree.  They evaluate and treat human disorders in the body. Whether the condition results from an injury, disease or other cause, they focus primarily on those individuals who have decreased function or impairment related to the musculoskeletal, neurological or cardiopulmonary systems. PTs evaluate the body function and apply the appropriate treatment to alleviate pain and improve physical function.

What do I need to bring?

Please bring any X-rays, MRIs, a list of medication and a prescription for physical therapy if you have them.  You may wish to bring comfortable clothing and gym shoes.

How many visits will I need?

The number of treatments will vary depending on the problem and individual. After a thorough assessment, the PT will discuss their treatment plan for the rehabilitation process.

Yes, gait analysis is included in your physical therapy coverage. If you do not have health insurance, please contact our office for self-pay rates.

Shorts and a T-shirt, dress shoes, running/walking shoes and orthotics (if you have them). Please do not apply lotion to your legs prior to your gait analysis.

Scheduling an appointment with a physical therapist just got easier. A new direct access law makes it possible for patients to schedule an appointment for physical therapy (PT) without having to visit a physician first. The Illinois Physical Therapy Act was recently amended to allow patients to have direct access to most licensed PT services. This change will mean a faster path to relief and recovery for patients with an injury or condition typically treated by a physical therapist. It can mean lower medical costs for patients. Physical therapists still involve the patient’s physician to assist with medical management, as needed.

Elevate Physical Therapy is contracted with most insurance carriers. Please call our office manager if you are having trouble finding our facility through your insurance carrier. We will be happy to call and verify benefits and confirm that you are covered.

Elevate Physical Therapy will work with your auto insurance carrier or worker’s compensation representative to facilitate your claims. Please bring all necessary claim information and contact information to your first appointment. Also, you can call in advance and we will contact your claim representative before your initial visit.

The Alter-G may be covered under your insurance and is part of your plan of care. You can also buy a series of sessions on the alter g.

Yes, Monday through Thursday we are open until 7:30pm. Friday we are open until 5:30pm and Saturday until 12:00 p.m
